Notice for Colorado Residents
Plans and policies offered by United Concordia Insurance Company do not include coverage of pediatric dental services as required under federal law. Coverage of pediatric dental services is available for purchase in the State of Colorado, and can be purchased as a standalone plan or as a covered benefit in another health plan. Please contact your insurance carrier, agent or Connect for Health Colorado to purchase either a plan that includes pediatric dental coverage, or an exchange-qualified standalone dental plan that includes pediatric dental coverage.

Discount Dental Products

The iDental Discount Plan is not insurance. Discount plans provide discounts for dental and vision services offered by providers who have agreed to participate in the discount plan. Discount plans do not provide insurance coverage and the discount plan organization does not pay providers directly. Discount plan members are required to pay providers directly for services subject to discounts under the plan. Discount Plan Organizations are United Concordia Life and Health Insurance Company, United Concordia Insurance Company and United Concordia Insurance Company of New York located at 4401 Deer Path Road, Harrisburg, PA 17011. Plans are marketed and administered by Detalplans.com. United Concordia does not direct content to Minors and does not intend to collect personal information from minors and therefore is not subject to the Child Online Privacy Protection Act.

Member agrees that he/she is responsible for protecting his/her password from compromise, release or discovery by any unauthorized person. If Member learns that his/her password has been compromised, Member should access the United Concordia website at www.unitedconcordia.com  to change his/her password. Passwords are changed using the following steps:

  1. Enter the Members section from United Concordia's home page
  2. Click on My Dental Benefits
  3. Select Forgot Password?
  4. Follow the steps to reset your password

Member agrees not to disclose his/her password to any third party in any manner.

 

Privacy/Treatment of Information or Data Transmitted to United Concordia

Personal information transmitted to United Concordia will be treated in accordance with the Privacy Policy, a copy of which can be accessed via the www.unitedconcordia.com website by clicking on the Privacy link.
