United Concordia is pleased to serve the dental insurance needs of PEBTF members. PEBTF and United Concordia are partnering because we care about the oral health of you and your family. Oral health problems often tax the immune system, which can expose you to greater risk of illness and infection. To help maintain a healthier smile and a healthier you, we encourage you to follow the advice of your dentist and the American Dental Association. Your dental plan emphasizes preventive care to help avoid problems commonly caused by delayed treatment.

About my plan - Your dental plan will be United Concordia's Dental PPO plan. United Concordia's Dental PPO plan provides you with access to more than 226,000 practicing locations nationwide through our Advantage Plus Network. Choosing one of our participating providers helps lower out-of-pocket costs and eliminates the need to fill out claim forms. If your dentist does not participate, you may still use your dentist, but your out-of-pocket expenses are likely to be higher.

How Does My Dental Plan Work? -

  • Members, including dependents, are free to use any licensed dentist; however, if the dentist is not participating with the Advantage Plus network, there may be more out-of-pocket expense as non-participating dentists can balance bill.

  • Services covered at 100%, such as preventive and diagnostic, are paid in full up to 100% of the Maximum Allowable Charge (MAC). You do not have to meet the annual deductible before payment is made on preventive and diagnostic services.

    Services covered by a coinsurance percent of less than 100% are subject to the deductible. For example, if a service is listed as having a 90% coinsurance amount, United Concordia will pay 90% of the MAC and you will be responsible for 10% of the MAC, after the $50 deductible has been paid for the treated family member. Please see your benefit summary for the coinsurance amounts for each service class.

  • There are no claim forms to submit when services are provided by a network participating provider.

  • A referral is not needed to see a specialist for care.
