How do I find out if my dentist participates in the Advantage Plus network?

Just click on Search for an Advantage Plus Dentist on the University of Virginia (faculty and staff) Clients' Corner page. You can also call Customer Service at 1-866-215-2354.


If my dentist does not participate in the Advantage Plus network, can I still see him/her?

Yes, you can receive care from any licensed dentist. If you choose to see a non-network dentist, you will be responsible for paying the dentist's full charge. You can save more by visiting a network dentist whose fees are limited to United Concordia Dental's maximum allowable charges.


How do I read my EOB?

Do I have to complete claim forms?

If you receive care from an Advantage Plus network dentist, you do not need to worry about claim forms; your dentist will take care of all the paperwork. If you receive care from a non-network dentist, you may have to complete and submit your own claim forms. Claim forms can be downloaded on your Clients' Corner page. Submit the completed forms to Dental Claims; P.O. Box 69421; Harrisburg, PA 17106-9421.


Does United Concordia Dental require predetermination of benefits?

Predeterminations are not required, although you should consider requesting that your dentist provide a predetermination before you begin treatment for services like crowns or dentures. That way you'll know whether or not a service is covered and how much you can expect to pay out of pocket.


How else can I estimate my out-of-pocket costs?

Members can use United Concordia's Cost Estimator, available on our website, to estimate their out-of-pocket costs for common covered procedures. Just log in to My Dental Benefits from your Clients' Corner page, choose the member who would be receiving the dental procedures and then select Cost Estimator.
