You should review these terms and conditions regularly as they may change at any time at the sole discretion of United Concordia.

Once the Member gains access to the information on the United Concordia website, he/she may browse the site and download materials contained on the site for his/her personal use only. However, no material or images from the website may be copied, reproduced, republished, transmitted, merged with any other works, or distributed in any way, for public or commercial purposes without the express written permission of United Concordia. Modifications to the contents of any portion of the website is a violation of United Concordia's copyright and other proprietary rights.

Member understands and agrees that the information he/she may access herein reflects information on file at the time of the inquiry and may be subject to retroactive and prospective changes as required by his/her plan design/administrator without notice. It is the responsibility of the Member to verify plan participation and coverage with his/her employer prior to receiving dental care services. United Concordia is not to be held responsible for personal Member liabilities resulting from retroactive cancellations of or changes to dental coverage by his/her employer.

Jurisdiction:

The United Concordia website is controlled and operated by United Concordia from its headquarters in the Commonwealth of Pennsylvania, United States of America. Any dispute arising from these terms will be resolved exclusively in the state and federal courts of Pennsylvania.

United Concordia does not direct content to Minors and does not intend to collect personal information from minors and therefore is not subject to the Child Online Privacy Protection Act.

Member agrees that he/she is responsible for protecting his/her password from compromise, release or discovery by any unauthorized person. If Member learns that his/her password has been compromised, Member should access the members' dental website to change their password. Passwords are changed using the following steps:

 - Enter the Members section of the Website
 - Click on My Dental Benefits
 - Select Forgot Password?
 - Follow the steps to reset your password
                
Member agrees not to disclose his/her password to any third party in any manner. Member also agrees not to use or attempt to use another’s account, service or system without authorization from United Concordia, or create or use a false identity on this site.

Privacy/Treatment of Information or Data Transmitted to United Concordia:

Personal information transmitted to United Concordia will be treated in accordance with the Privacy Policy, a copy of which can be accessed via the http://www.unitedconcordia.com website link.
