CA TPA Notice
United Concordia Companies, Inc. (UCCI) administers dental insurance for its affiliated named on your certificate of insurance. UCCI does business in CA as United Concordia Administrators.
Eligibility Notice
The following eligibility information can be used to determine the coverage period during which you and any dependents are enrolled in a United Concordia dental plan. While this information reflects the most recent information available, your employer reserves the right to retroactively cancel or change your coverage at any time. Before receiving care, it is always best to check with your employer to ensure that your coverage will remain effective during the desired treatment period. United Concordia does not reimburse members for services performed outside the coverage period due to retroactive plan cancellations.

Member acknowledges and understands that the information contained herein reflects information on file at the time of inquiry and may be subject to retroactive and prospective changes as required by his/her plan design/administrator without notice. It is the responsibility of the Member to verify plan participation and coverage with his/her employer prior to receiving dental care. UCCI will not be held responsible for personal member liabilities resulting from retroactive cancellations of or changes to dental coverage by his/her employer.
Certificate on Insurance Disclaimer
The Certificate of Insurance/Coverage “Certificate” includes any addenda, the Schedule of Benefits, the Schedule of Exclusions and Limitations, riders, and/or endorsements, if applicable. You must read the entire Certificate to determine coverage under your plan.
Cancelled Coverage
If this policy is cancelled, all users must create their own My Dental Benefits account and will have access to only their own information. Dependents under age 14 cannot create an account, but their information can be obtained by calling Customer Service at 1-800-332-0366.
